What is the pathogenesis of hypophosphatemia?

Phosphate stores are depleted during prolonged starvation. With initiation of nutrition, carbohydrates cause release of insulin. This increases cellular uptake of phosphate with subsequent reduced levels of serum phosphate. Insulin promotes intracellular production of adenosine triphosphate and 2, 3 diphosphoglycerate which further depletes the phosphate stores.
