What differentials do you have in mind?

Jacob isn’t drowsy, lethargic, mottled, or pale: he isn’t septic. This could be an early presentation of asthma or, based on his age and time of year, Bronchiolitis. Bacterial pneumonia is a possibility, but the pyrexia isn’t high enough and wheeze isn’t a stereotypical feature of bacterial pneumonia. There’s also croup (Laryngotracheobronchitis) another viral condition that affects the upper airways, but the characteristic features of barking cough, fever and inspiratory stridor are absent.
